Employer Reporting Requirements

Under the HCSO, covered employers must meet minimum health care spending thresholds based on the number of hours worked by eligible employees within San Francisco. In addition to making these expenditures, employers must submit an annual report detailing how they satisfied these obligations.

Although the filing deadline is typically April 30 of the following year, OLSE has granted a one-day extension for 2025 reporting, moving the due date to May 1, 2026. According to agency guidance, late submissions will not be accepted. Employers that fail to file on time may face penalties of $500 for each quarter of noncompliance.

Employers that were not subject to the HCSO or FCO during any quarter of 2025 are not required to submit the annual form. When accessing the online system, employers will first complete a brief questionnaire to determine coverage. If not covered, they will be notified that no filing is necessary.
